Oxybutynin antagonisiert kompetitiv die M1-, M2- und M3-Subtypen des muskarinischen Acetylcholinrezeptors. Oxybutynin hat als Calciumantagonist und Lokalanästhetikum auch direkte spasmolytische Wirkungen auf die glatte Muskulatur der Blase, jedoch in Konzentrationen, die weit über den klinisch verwendeten liegen.
